You will need to submit your order form by 4 April 2025. Items will be delivered to HQ after Easter.
Once you have submitted your order, we will send you an invoice for payment. Please note that your order won’t be confirmed until payment is received. If you can, please pay your invoice by 4 April (or soon after)
For friends of PMDLD, please ensure that you include the name of your referral from PMDLD as we will pass the items on to them, to pass on to you.
How this form works: please include your contact details so we can send you your invoice or get in touch if we have any questions. Please fill in item fields for items you are interested in ordering. (If you’re not interested in a particular item, please leave that item field blank). For items that you’d like to order, please include in the item field details such as qty, design, colour, size etc.
